To Schedule an Appointment
To make a medical or dental appointment, please be ready to provide the following information:
- Patient’s name and date of birth
- Your name (if you are calling for the patient)
- A current phone number where you can be reached during the day
- Whether the visit is urgent, routine or follow-up
- Patient’s insurance information or health coverage, if any
- If the patient will need an interpreter
- If you need a ride to your appointment, please let us know so we may help arrange transportation to and from our office.
- For medical appointments, name of the patient’s Primary Care Provider (PCP)
On the Day of Your Appointment
- Please plan to arrive 15 minutes before your scheduled appointment time. This will allow time for our reception staff to verify insurance, collect any co-pays, and update your file.
- You must bring your insurance card and copay.
- Bring us the medications you are taking. Many patients find it easy to simply bring all prescriptions and over the counter medications in a bag.
- The Center receives grants to help keep the cost of our services down. These grants allow us to offer more services. In order to get the grants, and keep the grants, we must report information about our patients.It includes:
- Income information
- Household size
- Race & ethnicity
- Age
- Housing information
Information on Dental Appointments
Because of a very high need and demand for dental services in Jackson, we have developed a scheduling policy that enables us to serve the greatest number of people.
New dental patients
Dental appointments for new dental patients are made on Mondays for the current week. Please call our dental office Monday morning to make an appointment if you are a new dental patient.
Returning dental patients
If you are a returning dental patient, you may call any day of the week to schedule a return appointment.
Emergency dental visits
If you or your child feel you have a dental emergency, like mouth pain or oral swelling during business hours, please call our office.
