Case Management Services
What is Case Management?
In a community health center, complex patient needs often become apparent during office visits and other interactions between patients and their providers. These needs often require more detailed intervention services than can be offered by the provider or their clinical staff due to time constraints or lack of resources.
Our Case Managers help to eliminate, decrease, or overcome barriers that prevent patients from getting services they need. They help patients achieve healthier lifestyles by addressing problems that limit their ability to comply with recommended treatments or medication orders from their doctor. Such barriers include:
Violent Relationships
Substance Abuse
Unemployment
Mental Illness
Poverty
Transportation
Not only do Case Managers help ensure patients are linked to appropriate resources within our own agency and in the community, they also serve as client advocates. Many times, patients are unable to seek services due to a lack of support or familiarity with processes. Our Case Managers also provide health education and general life skills education for our patients.
Who is eligible for Case Management services?
Anyone who is a CFH patient. Referrals are typically made by our doctors and midlevel providers. However, case management referrals can be made by any CFH employee who is concerned about one of our patients.
