What is HealthPay?

HealthPay is a discount program offered only to Center for Family Health patients.  You may receive medical and dental services for as little as $20 per visit.

How do I qualify for HealthPay?

You may qualify if you have no insurance or if you have large co-pays or large deductibles.  Discounts are based on income and family size.

What is covered by HealthPay?

HealthPay includes medical and dental office visits, prescriptions, lab testing, x-rays, and consulting services.

How do I sign up for HealthPay?

Stop by any of our offices. Our friendly staff will help you with your application.  You must bring proof of  income for the past month for anyone in your home over age 18.

What if you do not qualify for HealthPay?

For those patients who do not qualify for HealthPay, we do offer liberal payment programs. We turn no one away because of inability to pay.
