Billing/Patient Accounts
We will bill all insurance plans on your behalf. You are responsible for any amount that your insurance does not cover. If you are unable to afford your deductible or copay, our Financial Services Advocates may be able to help you.
In order for us to collect payment from your insurance, please make sure you present an up-to-date insurance card at each visit. Also, provide any changes in your address, phone number, employer, or Primary Care Physician. We need this information for your insurance company as well.
If you have questions about what your insurance covers, pre-authorizations, or if the Center for Family Health is participating or "In Network", you should contact your insurance company. They are the experts on your policy.
