The Board of Directors has adopted these Patient Rights and Responsibilities
Patients have the right to:
- considerate and respectful care without discrimination based on race, religion, color, sex, age, national origin, ancestry, marital status, parental status, disabilities or sexual orientation.
- know the names and positions of anyone involved in your care. Names will be displayed by official nametag and/or personal introduction.
- receive any necessary information from your provider about your diagnosis, treatment and prognosis using wording you can understand.
- participate in decisions involving your health care.
- receive any information you need from your provider giving permission for any procedures and/or treatments.
- refuse treatment to the extent permitted by law and to be told of the medical consequences of your action.
- every consideration of privacy regarding your own medical care.
- expect that all communications and records pertaining to your care will be treated as confidential.
- see your medical records.
- voice suggestions, complaints and grievances to appropriate levels of authority and receive a satisfactory response.
- receive information about the relationship of this health center to other health care and educational institutions as it pertains to your care.
- be told if the health center chooses to participate in human experimentation that will affect your care or treatment.
- expect reasonable continuity of care.
- receive and review an explanation of your bill, regardless of source of payment.
- know what health center rules and regulations apply to your behavior as a patient.
- Appropriate access to emergency services.
Patients are responsible for:
- Following the health center rules and regulations affecting your care.
- Providing a complete and accurate medical history.
- Asking questions when information about what you are expected to do or what kind of care you will receive is unclear.
- Following the recommendations and advice given to you in a course of treatment, if any.
- Expressing any concerns about your ability to follow recommended course of treatment.
- Being considerate of the rights of other patients, health center staff and property.
- Providing the health center with accurate and up-to-date information about your health care coverage and your ability to pay for services you receive.
