We will bill all insurance plans on your behalf. You are responsible for any amount that your insurance does not cover. If you are unable to afford your deductible or co-pay, our Financial Service Advocates may be able to help you.
In order for us to collect payment from your insurance, please make sure you present an up-to-date insurance card at each visit. Also, provide any changes in your address, phone number, employer, or primary-care physician.
If you have questions about what your insurance covers, pre-authorizations, or if the Center for Family Health is participating or “In Network,” you should contact your insurance company. Its employees are the experts on your policy.
