Primary Care

The Center for Family Health offers primary medical-care services through two departments – Pediatrics and Family Medicine at its downtown location, 505 N. Jackson St. Pediatrics serves newborns and children through the age of 17; Family Medicine serves patients of all ages. 

We encourage you to choose a provider you will see for most of your visits. You can peruse biographies and photos of the Center’s family medicine physicians, nurse practitioners, residents and physician’s assistants by clicking on Family Medicine Providers at right. Adult patients also can receive care at the Center for Family Health at LifeWays

You also can access biographies and photos of the Center’s pediatric providers by clicking the Pediatric Providers link at right.

The Center also operates four school health centers. All serve patients through the age of 21 — except the Northwest Community Health Center, which serves patients of all ages. Patients who usually see a provider at the downtown location also can use the school health centers.

Existing family medicine and pediatric patients can utilize the Walk-In Clinic at the downtown facility when they are unable to get an appointment with their regular provider.  


Appointments:

Call the Center at (517) 748-5500 for more information or to make an appointment.

You also can make an appointment at the office; same-day appointments sometimes are available. Please arrive 15 minutes before your scheduled appointments and bring your medication bottles to each appointment.

 

Prescription Refills:

Plan ahead: Allow three business days for a prescription refill. We do not refill medications over the weekend.

Use the Patient Portal to request a refill. If you are unable to access the portal, call 517-748-5500 to request a refill.

Please note: Prescriptions for pain medications and ADHD medication must be picked up at our office.

 

Hospital Care:

If you are admitted to the hospital, an Allegiance Health doctor will treat you during your stay. The Allegiance physician will keep us updated on your progress, and your Center provider will have access to all your hospital records and reports. Please make an appointment with your Center physician when you are discharged from the hospital.

If you are in need of medical services when our offices are closed, please call our after-hours Nurse Line at 517-780-3636. For medical emergencies, call 911 or go directly to the Allegiance Health Emergency Room.
